In order to make the best use of the time during your initial consultation appointment, please complete and submit the forms above before your first appointment. Forms submitted in the Patient Portal will automatically be available for review before your appointment and will be incorporated into your records. These forms will be reviewed by your provider prior to your first visit if they are submitted at least 2 business days before your appointment. Information completed less than 2 business days prior will be reviewed with you during your appointment. If unable to complete these forms within the Patient Portal prior to your appointment, you will be offered these forms during your initial appointment time which may encroach on face-to-face clinical time.
